An Introduction to Root Cause Analysis (RCA)

Course Overview

Within any Department in any Business mistakes, errors and issues arise, often resulting in significant impact including financial, brand damage and warranty claims. The only way to extract value from these mistakes is by implementing a Permanent Corrective Action (PCA); Root Cause Analysis is required to determine why the mistake / error occurred. Having witnessed many inadequacies in true RCA this training aims to bridge a knowledge gap and equip the attendees with the skills and comprehension to undertake effective RCA.

The course introduces RCA methods, including Fishbone Analysis, Reverse Fishbone Analysis and 5Ys. Examples covered are suitable for Engineering and non-Engineering professionals alike.

What is Root Cause Analysis (RCA)?

Discovering the root cause of problems and identifying solutions

Root cause analysis (RCA) is the process of discovering the root causes of problems in order to identify appropriate solutions. RCA assumes that it is much more effective to systematically prevent and solve underlying issues rather than just treating ad-hoc symptoms and putting out fires.
